The Client Care Manager is a leadership role for a proactive, take-charge LPN or RN.

The Client Care Manager enjoys building strong relationships and creating a positive client and family experience. The role combines oversight of client care, quality standards, team collaboration, and community engagement to help ensure consistent, high-quality support throughout each client’s care journey. 

Responsibilities 

• Help maintain and ensure strong standards of care  
• Manage and support day-to-day client care experience  
• Oversee and strengthen the care assessment and quality assurance process 
• Collaborate with the service team to support continuity of care 
• Monitor service quality, client satisfaction, and team performance 
• Maintain relationships with healthcare providers, senior living communities, and community partners 
• Participate in community outreach and referral activities as needed 
• Participate in the rotating after-hours escalation schedule 

What We’re Looking For 

• Current LPN or RN designation in good standing in Nova Scotia 
• 5+ years' experience in senior care, home care, community healthcare, or a related setting 
• Proactive, confident, and comfortable taking initiative 
• Supervisory experience is an asset 
• Strong communication, organization, and problem-solving skills 
• Professional, compassionate approach 
• Valid driver’s license and reliable vehicle
